Annual Health and Medical Records
Boy Scouts of America requires that Health and Medical Records be completed annually. Pack 167 chooses to do this at the beginning of each school year for ease of tracking compliance as a Pack. Completion of records applies to ALL participants (scouts, parents, and siblings)—in basic Scouting activities such as local tours and weekend camping trips less than 72 hours in duration.
• Download the form, fill it in digitally, and save for easy updating.
• Please include a copy of your current health insurance card and immunization records for all participants.
• Bring your completed, signed, and dated Health Forms to the Pack 167 office at an upcoming Pack event or Den Meeting.

Health Forms A, B:
Applies to ALL participants—in basic Scouting activities such as local tours and weekend camping trips less than 72 hours in duration. Does not require a physical by physician.
Health Forms A,B,C:
A pre-participation physical completed by a physician is required for events 72 hours or more. For our pack this would only include our summer Grayson Highlands trip for rising Webelos and AOL scouts. Boy Scout Troop requires part C form for all adventures.
